As Mr.GLN stated you can file with District collector PIO as the MP Lad funds will be spent through Collector of the district and if there are more than one district under the Lok Sabha constituency of the respective MP then you have to file separate applcations with both the PIOs of the districts concerned.I am of the personal opinion that the Lok Sabha secretariat can give how much funds were sanctioned by the respective MP from his MP Lads but may not have more details like Collectorate.PRASADJI HAS GIVEN YOU THE FORMAT NICELY YOU CAN USE AND FILE YOUR RTI.

There is a dedicated page website for information on mplad funds:

:: Welcome to MPLADS Home Page ::

There are also dedicated websites in some states for MLA Funds. Just google for them.

Lot of information is available on those websites. If you still cannot find the information you are looking for, then file a RTI application with PIO of Lok Sabha or Rajya Sabha.
