If an RTI Application dt Jun,2018  was found missing after first appeal hearing, simply either FAA or CPIO can ask for resubmission of the application.  The actual practice is PIO receives an application, and FAA receives an an appeal with application.  Such things may happen in rarest of the rare cases, calling the applicant and asking him to provide a copy and providing that information on top priority solves the issue.  If the PIO is not willing to provide the information, he may intimate applicant to provide a copy of application as the original application was misplaced.  An applicant can understand the position and this is not a very serious criminal or corruption issue to make dealing asst or CPIO accountable unless there are strong evidence against them.
