If a patient asks for disclosure of his/her medical records, is the doctor liable to provide the same?

should the original document should be provided to the patient or a xerox?

is it required to take a written request from the patient for the same?

Yes. Copy may be provided. It should be provided at the earliest or within 72 hours of application to the patient or his authorized relative or legal representative. Better take application from him.
